Thrive Support Services Inc. is seeking a Coordinated Family Support (CFS) Specialist in Martinez, CA, with work across Contra Costa and Alameda Counties.

The role focuses on comprehensive case management for adults with intellectual and developmental disabilities living at home, coordinating resources and promoting long-term independence. Responsibilities include intake, service planning, navigating public services, and collaboration with families and providers.

Qualifications

• Degree in human services or related field preferred, with relevant experience or combinations.
• Experience with developmental disabilities or related field is valued.
• Willingness to pass background checks and obtain required certifications.

Responsibilities

• Identify and coordinate supports for individuals to remain in the family home.
• Conduct intake, assessment, and service planning.
• Assist with navigation of public services (Medi-Cal, IHSS, Social Security).
• Facilitate education related to diagnosis and needs.
• Coordinate with providers to ensure quality of services.
• Assist with appointment scheduling and coordination.
• Identify transportation and backup services as needed.
• Support long-term planning for aging caregivers.
• Deliver training to promote independence and inclusion.
• Ensure culturally and linguistically appropriate services.
• Prepare and submit quarterly service progress reports.
