Qualifications

• Experience in coordinating access to resources for adults with I/DD.
• Experience with intake, assessment and service planning.
• Ability to navigate public services (Medi-Cal, IHSS, Social Security).

Responsibilities

• Identify and coordinate supports for individuals with I/DD to remain in the family home.
• Conduct intake, assessment, and service planning.
• Assist with navigation of public services (e.g., Medi-Cal, IHSS, Social Security).
• Deliver training to promote independence and inclusion.
• Prepare and submit quarterly service progress reports.
