Job summary

Thrive Support Services Inc. is seeking a Coordinated Family Supports (CFS) Specialist to work with adults with developmental disabilities in Contra Costa and Alameda Counties. The role involves small caseload case management, service coordination, and family collaboration to promote stability and independence.

You will coordinate access to resources, support long-term planning, and ensure culturally responsive services, with a semi-flexible schedule and an on-site work location in Martinez, CA.

Qualifications

• 2-4 year degree in human services or related field, or equivalent experience.
• Experience with developmental disabilities or related field preferred.
• Combination of education and experience acceptable.

Responsibilities

• Identify and coordinate supports for individuals to remain in the family home.
• Conduct intake, assessment, and service planning for clients.
• Assist navigating Medi-Cal, IHSS, and Social Security programs.
• Schedule and coordinate appointments and transportation as needed.
• Prepare quarterly service progress reports and document services.
