Job Description - Support Coordinator (25710)

Description

Full Time - 37.5 hrs per week (Rota is set up for a four day weekend once every two weeks)

We have an exciting opportunity for you to join our newly formed, highly respected ETAAS team, helping customers rebuild their lives after street homelessness or housing breakdowns.

Typical day as a Supported Housing Co-ordinator
  • Creating support plans with your customers and coordinating a range of different services to support our customers within a multi agency approach.
  • Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Attending regular case conference meetings ensuring the team is working collaboratively and utilising all additional support to give our customers the best chance of success.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Casual chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring
  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in maintaining and reviewing agreed plans, building trusting and lasting working relationships with our often complex and hard to reach customers.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
  • A highly organised and practical approach, to deal with housing management queries, personal needs and are confident to work with our contractors.
  • Experience in safeguarding, risk assessments and confidentiality, working collaboratively with colleagues across the organisation, and external agencies.

Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external candidates for this role. This remains under regular review.

Our Team

You'll be joining Calvin, our Senior Client Services Manager, who started as a Night Support Coordinator and is passionate about ensuring the most vulnerable receive person-centred care. You'll also work with Imogen who has joined the team originally as a Support Coordinator and has now progressed to the Client Services Manager role. Tracey, our Housing Management Coordinator, has supported customers across two Home Group projects - starting in mental health and becoming a key contact for housing-related matters. Together, they bring experience, dedication, and a shared commitment to making a real difference!

Job details
  • We work with our customers providing 24-hour support, which means you’ll be working on a rota basis. This role is based during the day working alternate weekends.
  • Where we’re flexible with you, we do need to meet the needs of our customers, but we'll give you the time you need to spend time doing the things that matter to you!
  • Able to use technology for creating and updating support plans, complete online learning and collaborate with colleagues.
  • You’ll need an Enhanced DBS check done and we pay for that.
