Qualifications

• Bachelor's degree in education, social work, psychology or human services related field is required.
• Minimum of two (2) years of experience completing and implementing Person Centered Individual Support Plans for clients receiving ID/DD Waiver Services is preferred.
• Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
• This position requires transporting clients or driving agency vehicles; candidates must have a good driving record with limited violations.

Responsibilities

• Collaborate with primary case managers, individuals, families, and community waiver providers to complete annual service reassessments.
• Lead shared planning processes to ensure comprehensive support for clients.
• Document and ensure that all exceptional medical and behavioral support needs are included in the individual's annual person-centered plan.
• Maintain compliance with all relevant regulations and standards, including DMAS, DBHDS Licensing, Office of Human Rights, and DOJ Settlement Agreement requirements.
