Job summary

Venture Community Services is seeking a dedicated Supports Case Worker to provide person-centered case management and coaching for individuals in Supported Collaborative Living and Independent Living Supports programs. The role focuses on helping clients achieve independence while ensuring services and documentation are in place.

This full-time position offers a $23/hour pay rate with paid training and a $1,000 sign-on bonus for full-time staff (and $500 for part-time).

Qualifications

• Bachelor's degree in social work, human services, psychology, rehabilitation, or related field is preferred; however, a bachelor's degree is not minimum requirement.
• High school diploma or GED required.
• Valid driver's license and reliable transportation are required; ability to communicate in English is essential.

Responsibilities

• Provide person-centered case management, coaching, and coordination to individuals served through Supported Collaborative Living (SCL) and Independent Living Supports (ILS).
• Develop routines and practical skills to help individuals live independently in their homes and communities.
• Assist with budgeting, bill payment, banking, and other money-management tasks while promoting informed, independent decisions.
