Job summary

Venture Community Services Inc. is seeking Supports Case Worker candidates to provide person-centered case management and coordination for individuals in Supported Collaborative Living and Independent Living Supports.

The role aims to promote independence and ensure required documentation is in place. The position offers full-time or part-time options with training and a $1000 sign-on for full-time (and $500 for part-time), plus competitive pay of $23 per hour.

Qualifications

• Bachelor's degree preferred but not required.
• High school diploma or GED required.
• Minimum age 21 years.
• Valid driver's license and reliable transportation.
• Experience in direct support or case management preferred.
• Ability to communicate effectively in English.

Responsibilities

• Build respectful relationships and provide person-centered support.
• Assist individuals to maintain independent living through routines and skills.
• Coach budgeting, bill payment, banking, and money management.
• Ensure safe, clean living spaces with household routines.
• Coordinate medical, dental, behavioral health appointments and follow-ups.
• Oversee medication systems per plan and agency policy.
• Keep service plans and records current and compliant.
• Assist with transportation planning and community resource access.
• Participate in planning meetings and coordinate supports.
• Communicate with individuals, families, guardians, and providers.
• Report health, safety, and service concerns promptly.
