Venture Community Services Inc. seeks a Supports Case Worker to provide person-centered case management and coaching for individuals in Supported Collaborative Living and Independent Living Supports. The role aims to maintain independence, coordinate services, and ensure compliance with plans, documents, and required records.

The position reports to the Individual Supports Coordinator and offers paid training, a Monday–Friday schedule, and a $1,000 sign-on bonus for full-time staff.

Qualifications

• Bachelor's degree in social work, human services, psychology, rehabilitation, or related field preferred.
• High school diploma or GED required.
• Minimum age 21 years.
• Valid driver's license and reliable transportation.
• Experience in direct support, case management, or independent living preferred.
• Ability to communicate effectively in English, verbally and in writing.

Responsabilités

• Build respectful, professional relationships with individuals served and provide support that reflects each person's strengths, choices, goals, and right to independence.
• Assist individuals in maintaining an independent living situation, including developing routines and practical skills.
• Provide coaching and support with budgeting, bill payment, banking, and other money-management tasks.
• Help individuals maintain a safe, sanitary, and organized home.
• Assist individuals with accessing medical, dental, behavioral health care and scheduling appointments.
• Oversee medication administration as per plan and policy.
• Document services, progress, and follow-up actions timely and accurately.
• Assist with transportation planning and community resource access.
• Participate in planning meetings and coordinate supports with the team.
