Job summary

Central New York Quest, Inc. is seeking a full-time Support Broker in Utica, NY to assist individuals who self-direct their services. This role requires OPWDD Broker Authorization, an associate degree, and a valid driver’s license.

You will help complete startup processes, monitor budgets, build Circle of Support, document services, and maintain records throughout the service period. The agency offers health insurance, dental/vision, generous paid time off, life insurance, a Simple IRA with a 2%

Qualifications

• OPWDD Broker Authorization is required.
• Associate degree in accounting or related field is required.
• Valid driver’s license and travel to participant homes/meetings required.

Responsibilities

• Assist the individual/family in startup process per OPWDD requirements.
• Monitor self-direction budget and expenditures.
• Help build Circle of Support and facilitate meetings.
• Develop a comprehensive Person-Centered Plan with Care Manager involvement.
• Ensure habilitation plans for self-hired staff are updated.
• Document self-directed services per Medicaid/OPWDD regulations.
• Track budget approvals and respond to information requests.
• Provide support around broker contracts and annual meetings.
