Job summary

Community Horizons is seeking a Support Broker to empower individuals to recruit and manage their own care teams, enhancing independence and control. You’ll partner with members and families to access tools, support, and guidance to navigate care systems and build strong caregiver relationships.

The role emphasizes advocacy, mentor-like support, and practical onboarding, with a semi-remote, flexible schedule in Shawano-area communities.

Qualifications

• Healthcare recruiting experience is required.
• 5+ years supporting individuals with disabilities or the frail elderly.
• Strong organization and communication skills.
• Passion for person-centered support and advocacy.
• Experience with self-directed or long-term care programs is a plus.
• A valid driver’s license and reliable transportation.

Responsibilities

• Attend referral meetings and conduct in-home visits with members and families.
• Help individuals recruit, screen, and hire their own caregivers or nurses.
• Support members through interviews, onboarding, and retaining quality staff.
• Mentor members on how to be successful employers, including managing schedules, communication, and staff expectations.
• Guide individuals through scheduling, timecards, and Electronic Visit Verification (EVV) systems.
• Assist members with Medicaid renewals, benefit applications, and navigating public assistance programs.
• Help individuals understand and navigate complex healthcare and long-term care systems.
• Maintain documentation and case notes while providing ongoing mentoring and support.

Your Role: Support Broker

As a Support Broker, you are more than a recruiter—you are a partner, mentor, and advocate helping individuals build the team that makes independent living possible.

You’ll work alongside members and families to ensure they have the tools, support, and confidence to lead their own care and navigate complex systems that impact their independence.

What You’ll Do
