Job summary

The Cuyahoga County Board of Developmental Disabilities is seeking a current Level 2 Support Administrator to join the Youth & Transition team. You will mentor new SSAs, provide ongoing support, coordinate services, and help develop person-centered plans that promote self-determination.

The role requires strong facilitation, problem solving, and the ability to travel across the region to support individuals and collaborate with providers.

Qualifications

• Must be a current Cuyahoga DD Support Administrator.

Responsibilities

• Mentor new SSAs and provide ongoing support to SSAs in practice.
• Assist SSAs with problem solving and service coordination.
• Facilitate development of person-centered plans to promote self-determination.
• Develop, authorize, and revise budgets based on assessed needs and ISP.
• Coordinate enrollment and paperwork related to Medicaid waivers as appropriate.
• Establish and maintain contact with providers and natural supports.
• Drive extensively within and beyond Cuyahoga County to perform duties.
