Job summary

The Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD) is hiring multiple full-time Support Administrators to coordinate person-centered plans, manage budgets, and help individuals and families access needed services.

Roles involve extensive community travel, collaboration with families, and ongoing plan reviews to ensure outcomes are met. A BA/MA and SSA certification are referenced in the salary and qualifications.

Qualifications

• Bachelor’s Degree in Special Education, Social Work, Psychology or related area.
• Experience providing service coordination and/or experience working in a DD program is highly recommended.
• The ability to obtain and maintain SSA certification with certification occurring within state statute time lines.
• Hold a valid driver's license and the ability to maintain an insurable driving record according to Cuyahoga DD policy.

Responsibilities

• Act as the primary point of coordination for the person-centered planning team using an assess/plan/review/revise process.
• Assist individuals and teams to identify issues, risks, and outcomes and revise assessments as needed.
• Facilitate development of person-centered plans and determine supports needed; revise as concerns or progress arise.
• Identify funding sources for supports via ISP and access cost-effective services.
• Establish and maintain contact with service providers to ensure coordinated services and community engagement.
