Job summary

Henry Ford Wyandotte Hospital is seeking a Case Manager to coordinate patient care, assess needs, and plan safe transitions across the care continuum.

You will advocate for patients and families, collaborate with clinicians, and optimize outcomes while ensuring high-quality, cost-effective care.

Qualifications

• Bachelor's degree in nursing or Master's degree in social work.
• Minimum 3 years of clinical experience.
• Valid, unrestricted State of Michigan license as RN or LMSW.
• Excellent verbal communication, written documentation, and interpersonal skills.

Role Overview

As a Case Manager, you’ll be a vital member of our patient-centered care team, ensuring seamless coordination and advocacy for patients and their families. Your role is to identify, assess, and plan care strategies that promote safety, quality, and cost-effective outcomes. You’ll facilitate transitions to the next level of care, manage critical diagnoses, and optimize health outcomes across the care continuum, making a direct impact on patient wellbeing.

Required Qualifications

• Bachelor's degree in nursing or Master's degree in social work.
• Minimum 3 years of clinical experience.
• Valid, unrestricted State of Michigan license as a Registered Nurse (RN) or Licensed Master Social Worker (LMSW).
• Excellent verbal communication, written documentation, and interpersonal skills.
• Strong problem-solving, analytical, and decision-making abilities.
• Proficiency with computer systems and healthcare software applications.

Preferred Qualifications

• Certification in Case Management (CCM) or Accredited Case Manager (ACM).
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
