As a Supervisor of Care Coordination, you will play a critical leadership role in guiding a team of care coordinators and ensuring the delivery of high‑quality, patient‑centered services. This position focuses on supporting social services and preventive care initiatives, with an emphasis on improving health outcomes for adult patients.
You will oversee daily operations, manage caseload distribution, and provide clinical and administrative supervision to staff. In collaboration with multidisciplinary teams, you will coordinate comprehensive care plans that address both medical and social determinants of health.
The role places a strong emphasis on prevention and patient education, including HIV prevention strategies such as PrEP and PEP, long‑acting injectable (LAI) treatments, and broader preventive care services. Experience or familiarity with sexual health education and adult patient care is highly valued.
