Job summary

Chapa-De Indian Health in Grass Valley is seeking an SUD RN Case Manager to coordinate care for patients with substance use disorders, including MAT participants. You will combine nursing expertise with care coordination to connect patients with services and community partners.

This role offers hourly pay, RN licensure required, BSN/MSN, and collaboration with a multidisciplinary team to support recovery.

Qualifications

• RN license (California)
• BSN or MSN degree
• Experience with Substance Use Disorders (SUD)
• Experience with MAT and behavioral health referrals

Responsibilities

• Provide nurse case management and care coordination for patients with SUDs including MAT.
• Assess patient needs using ASAM criteria and coordinate referrals for higher levels of care.
• Support induction, stabilization, and maintenance phases of MAT treatment.
• Build partnerships with community, county, state, and Indian Health Service agencies.
• Support SBIRT, Smoking Cessation, MAT grants, data collection and compliance.
