Job summary

Snoqualmie Indian Tribe seeks a Substance Use Disorder Professional/Case Manager to deliver culturally relevant SUD services to Tribal members and clients. You will conduct assessments, create treatment plans, provide counseling, and ensure continuity of care through case management and follow-up.

Responsibilities include leading tribal member case management, coordinating with external agencies, and documenting care in the electronic health record for efficient transition planning.

Qualifications

• Must have knowledge of SUD assessment, treatment planning, and case management.
• Experience with Tribal communities preferred.
• Knowledge of confidentiality laws (42 CFR Part 2, HIPAA) and WA codes.
• Ability to document care in electronic health records and coordinate with external agencies.

Responsibilities

• Provide SUD services including intakes, assessments, referrals, and treatment planning.
• Lead case management for Tribal members and coordinate with external facilities.
• Develop follow-up and aftercare plans to ensure continuity of care.
• Deliver counseling and education, maintaining thorough documentation in EHR.
