Job summary

Chapa-De Indian Health in Grass Valley is seeking a SUD RN Case Manager to combine your nursing skills with care coordination for patients in our Medication Assisted Treatment program. You will collaborate with providers, behavioral health teams, and community partners to assess needs using ASAM criteria and to guide induction, stabilization, and maintenance of MAT.

Qualifications

• BSN or MSN degree.
• Current RN license.
• Experience working with patients with substance use disorders.
• Strong patient assessment, care coordination, communication, and interdisciplinary collaboration skills.
• Knowledge of behavioral health and SUD treatment, including team-based MAT, along with proficiency in Microsoft Office and experience with eClinicalWorks or a similar EMR.

Responsibilities

• Provide nurse case management and care coordination for patients with SUDs, including patients receiving MAT.
• Assess patient needs using ASAM criteria, evaluate suitability for MAT, and coordinate behavioral health referrals and higher levels of care.
• Support patients throughout the induction, stabilization, and maintenance phases of MAT treatment.
• Build strong partnerships with community, county, state, and Indian Health Service agencies to strengthen continuity of care and patient resources.
• Support key SUD initiatives, including SBIRT, Smoking Cessation, MAT grants, program data collection, reporting, and compliance requirements.
