Job summary

Intermountain Centers in Arizona seeks a Substance Use Disorder (SUD) Navigator to support individuals with addiction and mental illness by coordinating outpatient services, screenings, brief interventions, and referrals to treatment and harm reduction services.

The role involves charting, maintaining confidentiality, collaborating with staff, visiting detox centers and hospitals, and advocating for clients to access continuous care.

Qualifications

• Minimum education is a high school diploma or equivalent.
• Behavioral health experience and/or substance abuse training preferred.

Responsibilities

• Maintain confidentiality of records related to member’s treatment.
• Assess members for substance use history and readiness to change.
• Counsel members individually or in group sessions to support recovery.
• Collaborate with staff to coordinate member care and treatment plans.
• Refer members to community resources or specialists as needed.
• Conduct community outreach and visit facilities to complete plans.
• Act as advocate to coordinate services and resolve emergencies.
