Job summary

Anew is an outpatient behavioral health provider dedicated to supporting individuals facing mental health and substance use challenges. We are seeking a SUD Case Manager to assess needs, develop service plans, and coordinate care across a multidisciplinary team.

The role focuses on coordinating services, advocating for clients, and promoting recovery. Flexible scheduling options and a supportive workplace are offered to help balance personal and professional commitments.

Qualifications

• Must have strong interpersonal communication with clients, families and professionals.
• Knowledge of behavioral health practices and community resources.
• Ability to work independently and as part of a team.

Responsibilities

• Coordinate and provide client-centered care that is timely and effective.
• Manage case assignments, draft service plans, review progress regularly.
• Advocate for clients in making informed decisions about their clinical status and treatment options.
• Collaborate with therapists, clients and families to meet behavioral health needs.
• Help clients achieve independence and develop daily living skills.
• Perform other duties as assigned and uphold documentation and billing practices.
