Job summary

The MetroHealth System is seeking a dedicated SUD Navigator to help patients and families maneuver the health care system and access needed resources. This on-site role involves coordinating care, conducting assessments, and linking patients to ongoing treatment and community supports.

Candidates should have a high school diploma (or GED) with strong communication, organizational, and cultural sensitivity skills, plus experience in health or social services.

• 5 years of medical health care experience.

Responsibilities

• Assist patients with substance use disorders and their families to navigate the health care system by removing barriers to care.
• Provide access to resources, transportation, and support, and enhance communication with health care providers.
• Conduct SUD assessments and linkages to ongoing treatment and community resources.
• Educate patients and communicate with clinical and administrative staff; monitor and report outcomes.
• Participate in meetings and activities; ensure follow-up with identified patients for up to 6 weeks or longer after discharge.
