Job summary

Qualifications

• Master's degree in a Human Services field is required.

Responsibilities

• Assess and identify client needs; develop treatment plans with clients.
• Facilitate outpatient groups and individual counseling.
• Coordinate discharge planning and follow-up.
• Document progress notes reflecting treatment objectives.
• Provide screening and evaluation for substance use and recommendations for treatment.
• Collaborate with other professionals on treatment plans.
