Job summary

Spring Leaf Solutions is a Washington, DC-based behavioral health organization seeking a Consumer-facing Clinical Care Coordinator for the Substance Use Disorder program. You will provide ongoing care coordination and clinical follow-up across a broad consumer panel, ensuring timely services and engagement.

Qualifications

• Must qualify as a practitioner authorized to provide Clinical Care Coordination under DC DBH Chapter 63.
• Active and unrestricted DC license/credential appropriate to the profession.
• Familiarity with person-centered treatment planning and ASAM levels of care.
• Ability to manage a large consumer panel with strong organizational skills.

Responsibilities

• Maintain regular contact with an assigned panel of consumers to assess engagement and progress.
• Review progress against treatment goals and identify barriers to treatment.
• Monitor services aligned with treatment plans and level of care.
• Coordinate required treatment plan reviews and updates based on progress and regulatory needs.
• Identify gaps in care and coordinate appropriate follow-up with clinical staff.
• Escalate clinical concerns to the appropriate clinician or care team member.
• Facilitate communication among counselors, prescribers, and other care team members.
• Support transitions between levels of care and coordinate referrals as needed.
• Conduct follow-up after significant events or changes in the plan of care.
• Document consumer contacts, needs, referrals, and interventions accurately.
• Monitor milestones, reassessments, and treatment plan updates.
• Participate in interdisciplinary case reviews and identify trends affecting care.
• Support quality improvement by identifying gaps in service delivery and continuity.
