Responsibilities

  • Maintain regular contact with an assigned panel of consumers to assess engagement and progress.
  • Review progress against treatment goals and identify barriers to treatment.
  • Monitor services aligned with treatment plans and level of care.
  • Coordinate required treatment plan reviews and updates based on progress and regulatory needs.
  • Identify gaps in care and coordinate appropriate follow-up with clinical staff.
  • Escalate clinical concerns to the appropriate clinician or care team member.
  • Facilitate communication among counselors, prescribers, and other care team members.
  • Support transitions between levels of care and coordinate referrals as needed.
  • Conduct follow-up after significant events or changes in the plan of care.
  • Document consumer contacts, needs, referrals, and interventions accurately.
  • Monitor milestones, reassessments, and treatment plan updates.
  • Participate in interdisciplinary case reviews and identify trends affecting care.
  • Support quality improvement by identifying gaps in service delivery and continuity.

Position Summary

Spring Leaf Solutions is seeking a consumer/patient facing Clinical Care Coordinator to provide ongoing care coordination and clinical follow-up for consumers enrolled in the Substance Use Disorder (SUD) program. This role maintains oversight of a broad consumer panel, routinely evaluating engagement, progress, care quality, and changing needs to help ensure consumers receive appropriate and timely services throughout their treatment journey.

The Clinical Care Coordinator serves as a central point of continuity between consumers and their multidisciplinary care teams, identifying gaps or concerns and coordinating appropriate follow-up with clinical staff.

Primary Roles & Responsibilities
  • Maintain regular contact with an assigned panel of consumers to assess treatment engagement, progress, needs, and overall experience with services.
  • Review consumer progress against established treatment goals and identify potential barriers to successful treatment.
  • Monitor whether consumers are receiving services consistent with their treatment plans and identified level of care.
  • Coordinate required treatment plan reviews and updates based on consumer progress, regulatory requirements, and changes in clinical needs.
  • Identify gaps in care, missed services, disengagement, changes in condition, or other concerns requiring clinical follow-up.
  • Escalate clinical concerns, safety issues, or changes in consumer needs to the appropriate clinician, Clinical Manager, Program Manager, or other care team member.
  • Facilitate communication among counselors, therapists, Recovery Support Specialists, assessors, prescribers, and other members of the consumer's care team.
  • Support transitions between levels of care and coordinate referrals to additional internal or external services when appropriate.
  • Conduct follow-up with consumers after significant treatment events, transitions, or changes in the plan of care.
  • Maintain accurate and timely documentation of consumer contacts, identified needs, referrals, interventions, and follow-up activities.
  • Monitor upcoming treatment milestones, reassessments, treatment plan updates, and other required clinical activities.
  • Participate in interdisciplinary case reviews and communicate trends or recurring barriers affecting consumer care.
  • Support quality improvement efforts by identifying recurring gaps in service delivery, consumer engagement, and continuity of care.
Required Qualifications
  • Must qualify as a practitioner authorized to provide Clinical Care Coordination under D.C. DBH Chapter 63, including an RN, LGSW, LICSW, LISW, LGPC, LPC, LMFT, APRN, psychologist, or physician, as applicable.
  • Must maintain an active and unrestricted District of Columbia license/credential appropriate to the profession.
  • Working knowledge of person-centered treatment planning, care coordination, and behavioral health documentation, with a familiarity with ASAM levels of care.
  • Strong organizational skills and demonstrated ability to manage and prioritize a large consumer panel.
  • Strong interpersonal and communication skills, including the ability to engage consumers with varying levels of treatment participation.
Preferred Qualifications
  • Master's degree in Social Work, Counseling, Psychology, Human Services, Behavioral Health, or a related field.
  • Experience working within SUD treatment programs or with individuals experiencing substance use disorders.
  • Experience within DC Department of Behavioral Health (DBH) and/or Medicaid-funded behavioral health programs.
  • Experience using electronic health record (EHR) systems and tracking consumer populations using reports or dashboards.
Work Setting

This is a hybrid position including a combination of field-based, in-office presence of at least two (2) days per week, and remote work. The position involves frequent telephone, virtual, and in-person interaction with consumers and ongoing coordination with members of the clinical team.

Performance Expectations

Success in this role is measured by consistent consumer outreach and follow-up, timely identification and escalation of care concerns, completion of required treatment plan activities, accurate documentation, and the ability to maintain visibility into the status and needs of a large consumer population.

The Clinical Care Coordinator is expected to maintain appropriate professional boundaries and operate within the scope of their credentials. Clinical decisions, diagnoses, and services requiring licensed clinical judgment must be referred to an appropriately credentialed clinician.
