Job summary

Anew Behavioral Health is seeking a SUD Case Manager to provide comprehensive coordination of care for clients facing substance use challenges. You will assess needs, develop individualized service plans, and collaborate with a multidisciplinary team to ensure effective, client-centered services.

The role focuses on advocacy, helping clients achieve recovery and greater independence, with flexible scheduling to accommodate personal availability.

Qualifications

• High school diploma or equivalent required.
• College degree in Human Services preferred.
• CPR certification preferred.
• NPI number/Ohio Medicaid ID required

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met.
• Facilitate clients in achieving personal independence and assist in developing daily living skills.
• Perform other duties as assigned and ensure documentation meets CARF/CMS standards.
