Job summary

Center for Family Services seeks an experienced Substance Use Counselor for Pathways in Williamstown, NJ. The role includes assessments, intake, and delivering individual and group counseling within an intensive outpatient framework.

You will create personalized treatment plans, provide case management, and maintain HIPAA-compliant documentation. Expected related licensure and trauma-informed care knowledge are highly regarded.

Qualifications

  • Master's degree in counseling, social work or another human services field is required or preferred.
  • CADC/LSW/LCADC preferred; active licensure or progress toward licensure is valued.
  • Knowledge of evidence-based treatment approaches and trauma-informed care is expected.

Responsibilities

  • Conduct intake assessments and assign level of care per ASAM criteria.
  • Provide individual and group counseling to clients in SUD programs.
  • Develop and update individualized treatment plans at least every 90 days.
  • Provide case management and connect clients with social determinants of health services.
  • Document all services in EHRs per regulatory requirements and agency policy.

Position Summary:

Center For Family Services is seeking an experiencedindividual for the Substance Use Counselor position in our PathwaysProgram. Pathways is an intensive outpatient and outpatient programproviding a personal support plan for recovery and relapse prevention. Theprogram helps clients experiencing problems with drugs and alcohol find thesupport they need to get their lives back on track. The Substance Use Counselor provides assessments and intake, individual and group sessions, and care management for clients with an SUDThecounselor is expected to be familiar with evidenced-based treatments and theprinciples of trauma informed care.

Duties and Responsibilities:

  • Ensures that appropriate face to face/telehealth intake assessments and accompanying paperwork are completed for each client utilizing NJSAMS and the electronic health record, AWARDS.
  • Assigns appropriate level of care according to ASAM criteria and gives appropriate referrals and assistance to clients not meeting criteria for treatment at CFS
  • Maintains a discharge/continuum of care plan which begins at the onset of treatment
  • Provides a minimum of (1) one hour per week of individual counseling and/or
  • Family-centered treatment services including family therapy and other supportive services as appropriate
  • Develops a treatment plan with the client that is updated as needed but no less frequently than every 90 days
  • Provides a structured schedule of treatment services
  • Provides case management to ensure access to social determinants of health
  • Provides documentation for all clinical and support services within established timeframes per Federal, State, County and agency regulations, policy and procedure
  • Conducts random urine drug screens (UDS) to be performed each month under chain of custody procedures.
  • Documents the identity of the referral source contact and communicates with the contact within 7 days of admission and provides regular progress reports regarding the client's status
  • Maintains a caseload per NJ Outpatient Licensure Standards
  • Adheres to HIPAA and 42 CFR, Part 2, regulations in all related client matters
  • Maintains professional credentials and participates in staff professional development trainings and workshops as mandated/required
  • Maintains weekly productivity standards of 70% of time in direct delivery service
  • Maintains show rate target of 71%
  • Assists the Clinical Director and/or Associate Vice President in monthly audits and report data as needed
  • Attends monthly/weekly supervision meetings and scheduled staff meetings

Requirements:

  • Preferred LCADC or LSW, LAC, or LMFT and actively working toward LCADC; at minimum CADC with H.S. Diploma (Associate's preferred)
  • Preferred Master's degree in counseling, social work or another human services field
  • Minimum master's level, academic year internship or one year post-graduate experience in substance use disorder counseling if not in possession of LCADC; two years experience in SUD counseling if not Master's level.
  • Is not presently engaged in drug or alcohol use that is likely to impair the ability to practice as a counselor with reasonable skill and safety. The term "presently" means at this time or any time within the previous 365 days.
  • Knowledge, training and application of evidence-based treatment and family systems
  • Knowledge of Trauma Informed practice and treatment and utilizes them when engaging with staff, clients, and stakeholders in the community
  • A valid NJ driver's license and insurance; CFS driving approval
  • Willingness to utilize clinical supervision to promote independent decision making and enhance system collaboration within a multidisciplinary team
  • Knowledge, skill and ability in DSM-V-TR, ICD-10, systems of care, risk factors, evidence-based assessment tools.
