Job summary

Health Care Resource Centers, a BayMark Health Services company, seeks a compassionate Substance Use Counselor to support patients in outpatient care. The role centers on individual and group counseling, crisis management, and treatment planning within a stable schedule (5:30–2:00).

You will conduct assessments, develop care plans, involve families, and coordinate services with a multidisciplinary team. Prior counseling experience with chemically dependent individuals is preferred; background

Qualifications

• Bachelor’s degree with 1+ year counseling experience.
• Master’s degree in Behavioral Science with 1+ year counseling experience.
• Certification plus supervision: CAC/CADC or CADC II / LADC II (MA).
• Experience with chemically dependent individuals is a plus.
• Must pass background check and drug screen.

Responsibilities

• Conducts initial assessment and evaluation interviews for service eligibility.
• Facilitates the development and review of the initial treatment plan.
• Completes Bio-psychosocial assessment.
• Provide individual and group counseling; direct service hours at scheduled intervals; coordinate services.
• Involve families and significant others in patient recovery when possible.
• Respond to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Other related duties as determined by the supervisor.
