Job summary

Health Care Resource Centers is seeking a compassionate Substance Abuse Counselor to guide patients through medically assisted programs and coping strategies in an outpatient setting.

The role includes conducting sessions, evaluating progress, and coordinating with doctors, nurses, and therapists to ensure optimal treatment outcomes. Collaboration and patient-centric care are emphasized throughout the program.

Qualifications

• Minimum HS Diploma; advanced degrees preferred.
• Previous experience with chemically dependent individuals a plus.
• Satisfactory drug screen and criminal background check.

Responsibilities

• Conduct initial assessments for service eligibility.
• Facilitate and review initial treatment plans with patients.
• Provide individual and group counseling; coordinate services across treatment.
• Complete bio-psychosocial assessments.
• Involve families in recovery when possible.
• Develop and participate in teaching projects for patients.
• Address patient grievances as needed.
• Develop discharge and aftercare plans.
