Job summary

Qualifications

• Assoc. Degree in Addiction or related field with minimum 3 years’ experience with SUD for Level I, or
• Bachelor’s degree in behavioral science with minimum 1 year of experience with SUD for Level II
• For individuals in recovery, must have maintained continuous recovery for two years at hire

Responsibilities

• Works with a treatment team to conduct comprehensive needs assessments
• Connects clients with resources, benefits, and entitlement programs
• Develops treatment plans for substance-use disorders aligned with policy and procedures
• Maintains clinical documentation in the EHR system in a timely manner
• Builds relationships with community partners and supports client goals
• Provides support to clients to achieve ISSP goals and objectives
• Stays informed about housing resources and eligibility for programs
