Job summary

Community Care Network in Rutland, VT seeks a Substance Use Street Outreach Specialist to engage individuals in the community who are struggling with substance use. The role emphasizes person-centered, trauma-informed, harm-reduction approaches and coordinates with the Adult Services Team.

You will work primarily in the field, meeting people where they are, and collaborate with the Rutland Free Clinic, Turning Point, and the Community Resource Team, with weekly supervision and interagency

Qualifications

• Must have a valid driver’s license with reliable transportation.
• Successful completion of a criminal history background check after hire.
• Experience in community outreach or mental health services is preferred.

Responsibilities

• Engage individuals in the community and participate in Outreach Care Team activities.
• Coordinate access to treatment services including MAT, detox, and crisis options.
• Collaborate with interagency partners to enhance service continuity.
• Maintain communication with medical leadership regarding medication changes and client status.
• Build rapport and engage clients respectfully to support recovery goals.
