Job summary

Elmcor Youth & Adult Activities Inc. is seeking a Nurse Practitioner in Queens, NY, to work with supervising physicians in expanding access to substance abuse treatments and coordinating care across health services.

The role focuses on patient engagement, clinical supervision, and collaboration with the Behavioral Health Team to ensure continuity of care. Some evenings and weekends may be required.

Qualifications

• Must hold NYS license to practice as a Registered Professional Nurse and NYSED certification to practice as a Nurse Practitioner.
• DATA 2000 “X” waiver.
• NYS Prescriptive Rights.
• Behavioral health background preferred.
• Must have computer skills.
• Must successfully complete a criminal background check and be drug free.

Responsibilities

• Participate in direct patient care.
• Monitor and interpret patients’ symptoms and needs.
• Provide immediate medical care in emergencies.
• Assess patients with a history of substance abuse disorders.
• Refer patients to appropriate medical/behavioral health facilities and follow up.
• Coordinate with Medical Doctor and staff for tele-practice services.
• Conduct HIV/Hep C screenings and referrals for high-risk populations.
• Provide care using Buprenorphine and/or IM Naltrexone when indicated.
• Induct and maintain patients on Buprenorphine.
• Document services and treatment plans in EHR systems.
