Noble Hearts HR Consulting Inc. is seeking a Nurse Practitioner in Queens, NY to lead addiction and behavioral health care on a full-time basis. The position collaborates with medical leadership to expand access to life-changing care and support for patients with substance use disorders.

Responsibilities include direct patient care, treatment planning, and coordination with a multidisciplinary team, with a focus on trauma-informed practices and community health impact.

Qualifications

• NYS Licensed NP with NYSED certification.
• DATA 2000 waiver (X-waiver) required.
• NYS Prescriptive Authority needed.
• Experience or interest in addiction medicine and behavioral health.
• Bilingual Spanish/English is a plus.

Responsabilités

• Provide direct, trauma-informed care to patients with substance use disorders.
• Assess, manage treatment plans, and prescribe medications (buprenorphine, naltrexone).
• Lead HIV/HEP C screenings and medical referrals for high-risk populations.
• Collaborate with a multidisciplinary team including mental health clinicians and care managers.
• Educate and advocate for health, wellness, and prevention.
• Support patients throughout recovery.

Location: Corona (Queens), NY
Salary: $150,000/year

Schedule: Monday through Friday; 6am - 2pm

What You’ll Do

In collaboration with our Medical Director and Behavioral Health leadership, you’ll be on the front lines of expanding access to life-changing care. Your role will include:

• Providing direct, trauma-informed care to patients with substance use disorders
• Performing assessments, managing treatment plans, and prescribing medication (Buprenorphine, Naltrexone)
• Leading HIV/HEP C screenings and medical referrals for high-risk populations
• Working with a compassionate, multidisciplinary team including mental health clinicians, peers, and care managers
• Delivering education and advocacy for health, wellness, and prevention
• Supporting patients every step of their recovery journey
