Elmcor Youth & Adult Activities Inc. in Queens, NY seeks a Clinical Counselor for a Mobile Unit to provide community-based counseling for chemical dependency and mental health. The role reports to the Program Manager and involves collaboration with a treatment team.

Key duties include intake assessments, treatment planning, and both individual and group counseling, using evidence-based practices to support clients and address safety and medical necessity.

Qualifications

• BS/BA in a human services field and CASAC with 2 years’ experience working with applicable populations who have substance abuse/co-occurring disorders.
• Associates Degree and CASAC-T and 4 years applicable experience.
• Must have computer skills.
• Must successfully complete a criminal background check, obtain clearance from the Office of Children and Family Services and be drug free.

Responsibilities

• Collects and analyzes assessment data including substance use, mental health and co-occurring disorders.
• Completes intake process and psychosocial assessments and makes treatment recommendations.
• Develops initial and comprehensive individualized treatment plans with measurable objectives.
• Provides individual and group counseling based on treatment plan and participant needs.
• Conducts evidence-based practices (EBPs) such as CBT, Motivational Interviewing, Matrix Model, Trauma Focused CBT.
• Identifies urgent mental health conditions and makes referrals to emergency or psychiatric services.
• Addresses trauma indicators and maintains safety and trust in the professional relationship.
• Addresses social needs and utilizes referrals for specialty care or hospitalization.
• Discharges planning and liaises with courts, parole, probation, ACCESS, etc.
• Documents counseling sessions, assessments, interventions, and billing information.
