Job summary

Applegate Recovery is an addiction treatment program that provides medically supervised services for adults in various modalities and settings. The licensed counselor will support patients enrolled in Medication Assisted Treatment for opioid use disorder, using evidence-based practices and a trauma-informed approach to develop coping techniques and symptom management skills.

• Must be LPPC, LICDC, or LICDC-S licensures (LCD CII, LCDCIII with supervision).
• Satisfactory drug screen and criminal background check.

Responsibilities

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
• Conduct initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond to patient grievances and complaints as needed.
• Develop aftercare and discharge plans.
• Other related duties as determined by supervisor.
