Job summary

Applegate Recovery in Dayton, OH is seeking a full-time Licensed Counselor to support adults enrolled in Medication Assisted Treatment for opioid use disorder. You will apply evidence-based, trauma-informed care and help patients develop coping skills and an individualized plan of care with a multidisciplinary team.

The role involves assessments, treatment planning, responding to concerns, aftercare and discharge planning.

Qualifications

• Licensed professional counselor credentials and licensure requirements.
• Experience delivering individual and group counseling.
• Experience supporting patients in MAT for opioid use disorder.

Responsibilities

• Provide individual and group counseling; coordinate services throughout treatment.
• Conduct initial assessment and determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish and review individualized treatment plans.
• Respond to patient grievances and complaints.
• Develop aftercare and discharge plans.

Full Time - Licensed Counselor

Applegate Recovery is an addiction treatment program that provides medically-supervised services for adults in a variety of modalities and settings. Individualized coping techniques and symptom management skills that enable then to experience improved quality of life. Through collaboration with doctors, nurses and a multidisciplinary team, the counselor will play a vital role in evaluating patient progress and developing an individualized plan of care.

Essential Job Functions:

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
• Conducts initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond, as needed, to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Other related duties as determined by supervisor.

Qualifications:

• Must be an LPPC, LICDC, or LICDC-S licensures (if there is no counseling supervisor) LCDCII, LCDCIII with counseling supervision
• Satisfactory drug screen and criminal background check.
