Job summary

AppleGate Recovery, a Baymark Health services company, is seeking a compassionate Full Time Licensed Substance Abuse Counselor to join our outpatient clinic team.

You will provide individual and group counseling for patients enrolled in Medication Assisted Treatment for opioid use disorder, and collaborate with doctors, nurses and a multidisciplinary team to develop individualized plans of care.

Qualifications

• Must hold an appropriate Kentucky license (LCADC/CADC, LCSW/CSW, LMFT/LMFTA, LPCC/LPCA).
• Experience with chemical dependency and MAT for opioid use disorder is preferred.
• Must pass drug screening and background check.

Responsibilities

• Provide individual and group counseling; coordinate services per patient need and regulations.
• Conduct initial eligibility assessment and complete bio-psychosocial evaluation.
• Establish and review individualized treatment plans with each patient.
• Address patient grievances and develop aftercare and discharge plans.
• Contribute to a multidisciplinary treatment team.

Compassionate Full Time Licensed Substance Abuse Counselor Needed!

Position Overview

The licensed counselor will be responsible for offering support and assistance to patients enrolled in Medication Assisted Treatment for opioid use disorder. Utilizing evidence-based practices and a trauma-informed approach, the counselor will help patients develop individualized coping techniques and symptom management skills that enable them to experience improved quality of life. Through collaboration with doctors, nurses and a multidisciplinary team, the counselor will play a vital role in evaluating patient progress and developing an individualized plan of care.

Responsibilities

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices, and state regulations; coordinate all services throughout treatment.
• Conducts initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond, as needed, to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Other related duties as determined by supervisor.

Qualifications

• Must hold at least one of the following in the state of Kentucky:
  • Licensed Clinical Alcohol and Drug Counselor (LCADC)
  • Certified Alcohol and Drug Counselor (CADC)
  • Licensed Clinical Social Worker (LCSW) or Clinical Social Worker (CSW)
  • Licensed Marriage and Family Therapist (LMFT) or Licensed Marriage and Family Therapist Associate (LMFTA)
  • Licensed Professional Clinical Counselor (LPCC) or Licensed Professional Counselor Associate (LPCA)
• Prior experience working with individuals affected by chemical dependency
• Must demonstrate a positive, non-judgmental, and accepting attitude toward the patient population.
• Successful completion of a drug screening and criminal background check is required
