Qualifications

• Bachelor's degree in Social Work or related field required; work experience may not substitute.
• ORAS Risk/Needs/Responsivity Assessment Certification or VPRAI-R Pretrial Risk Assessment required.

Responsibilities

• Conducts evidence-based screening and assessment to identify behavioral health needs and barriers.
• Develops individualized care plans addressing medical, psychological, housing, and social needs.

• Facilitates referrals for further clinical assessment as indicated.
• Coordinates discharge planning and aftercare linkage with community resources.
