Qualifications

• Current NJ RN license is required.
• 2–3 years in subacute/rehabilitation or long-term care preferred.
• At least 1 year in a supervisory/management role preferred.
• Strong knowledge of NJ DOH and CMS regulations.
• Excellent leadership, organizational, and communication skills.
• Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Clinical Leadership: direct and supervise nursing care on the subacute unit.
• Admissions, transfers, and discharges with thorough assessments and documentation.
• Collaborate to develop and monitor individualized care plans.
• Monitor progress and prevent rehospitalization as needed.
• Staff Management: lead RNs, LPNs, and CNAs; manage scheduling and assignments.
• Conduct staff evaluations, provide feedback, and support development.
• Regulatory Compliance & QA: ensure NJ DOH and CMS compliance; participate in audits and QAPI.
• Maintain documentation for care, incidents, and reporting.
• Resident & Family Relations: be primary contact for care needs and concerns.
• Administrative Duties: assist with budget, supplies, and metrics reporting.
