Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking an experienced RN Unit Manager to join our nursing leadership team. This full-time role oversees daily clinical operations on an assigned unit, ensuring high-quality, resident-centered care and regulatory compliance.

You will mentor RNs, LPNs, and CNAs, participate in care planning, admissions, discharges, and QA initiatives while promoting a collaborative culture focused on excellence in resident care.

Qualifications

• Current and active RN license in New Jersey.
• Leadership or supervisory experience in a SNF or subacute rehab setting preferred.
• Strong knowledge of long-term care regulations and clinical practices.
• Excellent communication, leadership, and organizational skills.
• Ability to effectively manage multiple priorities in a fast-paced environment.
• Strong problem-solving and critical-thinking skills.
• Passion for delivering exceptional resident-centered care.

Responsibilities

• Supervise and manage the day-to-day operations of the assigned nursing unit.
• Lead, mentor, and support RNs, LPNs, and CNAs.
• Ensure residents receive high-quality, individualized care in accordance with physician orders and care plans.
• Monitor clinical documentation and maintain compliance with all federal, state, and local regulations.
• Conduct daily rounds and address resident and family concerns promptly and professionally.
• Collaborate with the interdisciplinary team to achieve the highest standards of care and resident satisfaction.
• Assist with admissions, discharges, and care plan meetings.
• Participate in quality assurance and performance improvement initiatives.
• Ensure survey readiness and assist during state and federal inspections.
• Promote a positive, team-oriented culture focused on excellence in resident care.
