Job summary

Atlas Healthcare, LLC is seeking a Subacute Unit Manager to lead the subacute nursing unit, coordinating care and supervising RNs, LPNs/LVNs, and CNAs to ensure safe, compassionate, high-quality care for residents recovering from acute illness or surgery.

The role collaborates with physicians, rehab, social services, dietary, pharmacy, residents, and families to promote positive outcomes, oversee assessments and care plans, and maintain regulatory compliance.

Qualifications

• Current RN license in good standing in the state of employment (New Jersey).
• Associate Degree in Nursing is required; BSN preferred.
• Experience in nursing leadership or supervision, preferably in subacute/rehabilitation/acute care.
• Strong clinical assessment and critical-thinking skills.
• Knowledge of regulations, infection prevention, and quality standards.
• Experience with EMR and clinical documentation systems.
• Excellent communication, organization, delegation, and time-management skills.
• Ability to lead a multidisciplinary nursing team.
• BLS certification required or obtained within required timeframe.

Responsibilities

• Provide day-to-day clinical leadership and oversight of the subacute nursing unit.
• Supervise, support, and evaluate nursing staff (RNs, LPNs/LVNs, CNAs).
• Ensure residents receive individualized, evidence-based care per care plans and orders.
• Monitor changes in condition and coordinate timely assessments and interventions.
• Coordinate admissions, transfers, discharges, and transitions of care.
• Oversee documentation including assessments, care plans, progress notes, and MARs.
• Participate in interdisciplinary care planning and communicate goals to the team.
• Monitor patients with complex needs (IV therapy, wound care, etc.).
• Ensure follow-up of labs, tests, orders, and consults.
• Promote communication among residents, families, and staff.
• Assist with staffing, scheduling, and coverage to maintain safe levels.
• Conduct rounds to ensure quality, safety, and compliance.
• Provide coaching, orientation, and ongoing education to staff.
• Address performance and participate in evaluations and disciplinary processes.
