Job summary

Northgate Health Care Facility is seeking an RN Unit Coordinator of Subacute to oversee the clinical operations of an assigned nursing unit, ensuring high-quality resident care, regulatory compliance, staff supervision, and interdisciplinary coordination.

Requires an Associate Degree in Nursing and NYS RN license; at least 1 year of long-term care experience and 3-5 years of supervisory experience preferred.

Qualifications

• Associate Degree in Nursing is required; Bachelor's degree preferred.
• NYS RN license required.
• 1 year of RN experience in long-term care required.

Responsibilities

• Oversee the clinical care and daily operations of the Subacute unit.
• Supervise nursing staff, nursing assistants, rehabilitation aides, and unit support staff.
• Conduct resident assessments and ensure care plans are implemented, updated, and followed appropriately.
• Monitor resident conditions and communicate changes with providers, families, and interdisciplinary team members.
• Review provider orders, laboratory results, incident reports, and quality indicators.
• Participate in care plan meetings, family meetings, QAPI activities, and interdisciplinary rounds.
• Coordinate admissions, discharges, transfers, outside appointments, and resident services.
• Ensure accurate documentation, medication management, and regulatory compliance.
• Support staff education, orientation, coaching, and performance management activities.
• Monitor infection control, immunization tracking, resident safety, and quality improvement initiatives.
• Address resident, family, and staff concerns while promoting a positive care environment.
