Job summary

Masonic Villages of Pennsylvania is seeking a Sub-Acute RN/Rehab RN to join our 23-bed unit on a long-term care campus. This full-time role offers evenings or nights with a pay range of $33.00 to $45.75 per hour, plus night premiums of $2.50 and weekend differentials, and uses PCC for seamless documentation.

Join a team dedicated to helping residents recover and return to home or community settings. Must hold PA RN licensure, CPR, and prefer acute or subacute experience.

Qualifications

• RN graduate from an accredited School of Nursing.
• Current RN licensure by the PA State Board of Nursing.
• CPR Certification required prior to or upon employment.
• Preferred experience in acute or subacute care.

Responsibilities

• Monitors nursing care and ensures continuity per I.D.C.P. and orders.
• Assesses patient condition and care needs during transition.
• Ensures medications and treatments are provided as ordered.
• Maintains documentation in medical records per standards.
• Assists physician during rounds and procedures.
• Provides direct resident care and transports to activities as needed.
• Responds to call lights and performs glucometer, EKG, and related tests.

Our Sub-Acute RN/Rehab RN Is Responsible For The Following

• Monitors nursing care by visiting residents routinely to insure continuity and continual evaluation of care as related to I.D.C.P. and physician's orders.
• Assesses transitional care patient's condition and nursing care needs.
• Assures that medications, treatments and care are being provided as ordered or specified on I.D.C.P.
• Maintains appropriate documentation of nursing observations, including follow-up documentation in medical record in accordance with acceptable medical standards of practice.
• Prepares equipment and assists physician during rounds or special treatments and examinations of residents.
• Provides direct resident care and transports residents to activities and appointments as time permits.
• Answers call light promptly. Performs glucometer readings, EKGs and other testing as needed.

What You Need As a Sub-Acute RN/Rehab RN

• RN Graduate from an accredited School of Nursing
• Current RN licensure by the PA State Board of Nursing
• CPR Certification prior to or upon employment required
• Prefer previous acute or subacute experience
