Job summary

Specialized Education Services, Inc. in Hartford, CT is seeking a School Transition Coordinator to support post‑secondary planning for students transitioning from high school.

You will collaborate with teachers, families, and community partners to design and implement transition plans and coordinate campus visits and informational sessions. You will monitor FAFSA and related requirements, prepare reports for leadership, and provide staff training on best practices in student transitions.

Qualifications

• Bachelor’s degree in Education, Counseling, Social Work, or a related field is required.
• Master’s degree is preferred.
• Experience in educational administration, counseling, or student support services is desirable.

Responsibilities

• Transition Planning & Implementation: Assist graduating students in planning their transition from high school to post‑secondary education and/or employment.
• Establish ACT 158 Pathway plan and help navigate district requirements.
• Organize orientation sessions, campus tours, and informational meetings for students and families.
• Develop and implement comprehensive transition plans for students moving between schools or programs.
• Collaboration & Communication: Plan outside agency engagement to discuss opportunities after graduation.
• Work with educators, counselors, and staff to tailor transition support.
