Job summary

Milestones Day School and Transitions Program is seeking a Transition Planning Specialist to join the Post High School team in Waltham, MA. You will support middle and high school students (grades 7–12) with autism spectrum disorders and related needs, guiding transition planning for education, employment, and independent living.

Work closely with educators, clinicians, and staff to integrate transition skills into academic and therapeutic programming, while coordinating with the Post Secondary

Qualifications

• Bachelor's degree required; Master's strongly preferred.
• Transition Leadership Certificate preferred.

Responsibilities

• Conduct formal and informal transition assessments for grades 9–12 to support individualized postsecondary goals.
• Share assessment findings with IEP teams to guide transition services and goals.
• Participate in IEP meetings, re-evaluations, and parent transition meetings as appropriate.
• Identify student strengths, interests, and needs related to employment, education, and independent living.
• Coordinate with case managers and staff to integrate transition skills into instruction and activities.
• Oversee and coordinate Senior Seminar planning, summer transition programming, and related logistics.
