Job summary

Specialized Education Services, Inc. (SESI), a division of FullBloom, seeks a School Transition Coordinator to guide students from high school to post-secondary paths or employment.

The role involves collaborating with teachers, counselors, families, and community partners to create a structured transition experience, monitor FAFSA/Xello, and develop individualized plans across grade levels, with ongoing staff development.

Qualifications

• Bachelor’s degree in Education, Counseling, Social Work, or a related field (Master’s degree preferred).
• Experience in educational administration, counseling, or student support services.
• Strong organizational, communication, and interpersonal skills.
• Ability to work with diverse groups including students, families, and school personnel.
• Familiarity with student support services or related areas is an asset.

Responsibilities

• Assist graduating students with planning transitions from high school to post-secondary education or employment.
• Organize orientation sessions, campus tours, and informational meetings.
• Develop transition plans for students moving between schools or programs.
• Coordinate meetings with community partners and outside agencies.
• Monitor FAFSA and Xello requirements and evaluate program effectiveness.
• Prepare regular reports for school leadership.
• Provide staff training on best practices in student transitions.
