Job summary

Specialized Education Services, Inc. (SESI), a division of FullBloom, is seeking a School Transition Coordinator to guide students from high school to post-secondary paths, including education or employment.

You will partner with teachers, counselors, families, and community partners to plan and support each student’s transition. The role emphasizes individualized planning, coordinating ACT 158 pathway requirements, organizing orientations, and tracking FAFSA/Xello progress.

Qualifications

• Bachelor's degree in Education, Counseling, or related field.
• Experience in educational administration or student support services.
• Strong organizational and communication abilities.
• Ability to work with diverse students, families, and staff.
• Familiarity with special education practices is a plus.

Responsibilities

• Support post-secondary planning and transition from high school.
• Create ACT 158 Pathway plans for individual students.
• Organize orientation sessions, campus tours, and meetings.
• Develop transition plans for students moving between schools.
• Coordinate meetings with community partners and agencies.
• Monitor FAFSA and Xello requirements.
• Prepare regular reports for school leadership.
• Provide training on transition practices for staff.
