Job summary

Specialized Education Services, Inc. (SESI), a division of FullBloom, seeks a School Transition Coordinator to guide students from high school to post-secondary paths or employment.

Qualifications

• Bachelor’s degree in Education, Counseling, Social Work, or a related field (Master’s degree preferred).
• Demonstrated experience in educational administration, counseling, or student support services.
• Strong organizational, communication, and interpersonal skills.

Responsibilities

• Assists graduating students in planning their transition from high school to post-secondary education and/or employment.
• Establishes ACT 158 Pathway plan for each student and assists navigation to meet requirements.
• Organizes orientation sessions, campus tours, and informational meetings for students and families.
• Develops comprehensive transition plans for students moving between schools or programs.
• Plans with external agencies to discuss opportunities after graduation with graduating seniors.
• Works with educators, counselors, and staff to tailor transition support to individual needs.
• Serves as main contact for families during transition periods and provides resources.
