Job summary

The Case Manager - Student Assistance Program at Gloucester, part of Beth Israel Lahey Health, provides preventive counseling and coordinated services for students. Under supervision, you engage with schools, families, and community partners, delivering outreach, education, and triage support to promote youth wellbeing.

Part-time role (20 hours/week) with afternoon and evening shifts, requiring driving and transportation to outreach sites.

Qualifications

• Experience working with youth and families and substance use issues.
• Ability to complete documentation electronically and timely.
• Must have a valid driver's license, good driving record, reliable vehicle, and be willing to transport persons served.

Responsibilities

• School Counseling: provide preventive individual/group support; collaborate with school staff and parents; assess needs and provide referrals.
• Prevention Education: deliver classroom presentations and campaigns on youth substance use and mental health.
• Community Based Services: plan and facilitate outreach events; be available evenings and weekends; transport students as needed.
• Administration: weekly supervision; maintain logs; submit paperwork and outcomes measurements.
• Public Representation: represent the program at health fairs, coalitions, and special initiatives.
• Other duties as assigned.
