Beth Israel Lahey Health in Gloucester is seeking a part-time Case Manager for the Student Assistance Program. The role provides short-term school-based prevention services to youth at risk for substance misuse, helping them build coping skills and connect with resources.

Under supervision, you will coordinate with school staff, families, and community providers; deliver prevention education, maintain documentation, and participate in program evaluation.

Qualifications

• Bachelor degree in counseling, psychology, social work or related field.
• Experience working with youth, families and substance use issues.
• Ability to complete written documentation timely and effectively.
• Must have valid driver’s license and reliable vehicle for transportation.

Responsibilities

• Provide preventative school-based individual and group support to students.

• Plan and facilitate community events; some evenings/weekends may be required.

Case Manager - Student Assistance Program (Gloucester)

The Case Manager provides short term school-based individual and group prevention services to youth who present with risk factors to substance misuse. Helps youth served learn and practice coping skills, increase connections to community/school resources and improve the quality of their support network. Will complete all necessary outcome tools and collect data as needed to comply with expectations of program contract through BSAS.

Responsibilities

• School Counseling: Under supervision of the Senior Case Manager, provides preventative individual and/or group support to students; coordinates closely with school support staff, faculty, and administration to deliver services; collaborates with parents/caregivers; assesses academic and family needs; provides referrals to support services in school and to community providers as needed; provides triage support as needed.
• Prevention Education: facilitates prevention education activities via classroom presentations, assemblies, campaigns, etc. that address youth substance abuse, suicide prevention and other topics related to mental health.
• Community Based Services: assists in the planning and facilitation of prosocial student and family events based around recreational activities, crafts, excursions. Available at least one evening per week and occasional weekends to provide services or facilitate events in outreach settings including family homes or community locations. Available to transport students as needed.
• Administration: attendance at minimum weekly 1:1 supervision and other supervision/meetings as requested; checks agency email daily and available by telephone during school hours daily; keeps essential program tracking logs up to date; submits required paperwork as scheduled and requested; regularly submits and tracks outcomes measurements; conducts satisfaction surveys as needed.
• Public Representation: serves as a representative for the division, program, and stakeholders through the following venues as assigned; community and school health fairs, focus groups, coalitions, and other special initiatives to increase advocacy for attention to youth substance abuse and/or promote the program and stakeholders.
• Other duties as assigned

Job Qualifications

• Bachelor Degree in Counseling, Psychology, Social Work or a related field; comparable experience will be considered.
• The individual must have experience and training in working with youth, families and substance use issues. A working knowledge of computers and ability to complete documentation electronically is required.
• In addition the ability to complete written documentation timely and effectively.
• Must be able and willing to transport persons served. As such, all qualified candidates must have a valid driver's license, good driving record, and reliable vehicle. Travel and an ability to meet persons served in a variety of outreach settings is required.

e:Part-time, 20 hours (2/3 days per week) - Afternoon & Evenings. Benefit Eligible.

Pay Range

$22.00 - $26.82

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
