Qualifications

  • High School diploma or GED; Associate's degree preferred.
  • Two years of experience in social work or related field.
  • Fluency in language of the target population preferred.

Responsibilities

  • Gathers attendance and referral data via intake interviews and contacts students/parents.
  • Runs and analyzes reports to flag attendance, discipline, or other issues.
  • Collaborates with school and district staff to determine interventions.
  • Schedules appointments with counselors or social workers for interventions.
  • Uses case management to monitor interventions and follow up.
  • Conducts screening interviews for those referred to groups.
  • Collects and shares referrals for campus support groups with coordinators.
  • Maintains confidential student files and data for reports.
  • Notifies families of contact outcomes and documents attempts.
