The ideal candidate will have 3-5 years of nursing experience with 1-3 years in navigation or care coordination, and hold an RN license with BCLS certification.

Qualifications

• 3-5 years of clinical nursing experience required.
• 1-3 years of experience in patient navigation, care coordination, or case management preferred.

Responsibilities

• Serves as a primary point of contact for patients, families, and caregivers from diagnosis through the end of treatment, providing ongoing support.
• Assesses patients' medical, emotional, and socioeconomic needs, addressing barriers to care and coordinating solutions to improve adherence and outcomes.
• Provides patient education, addressing concerns about treatment, potential side effects, and management strategies to improve understanding and quality of life.
• Facilitates communication between patients, caregivers, and healthcare providers, ensuring timely information sharing.
• Acts as a patient advocate, collaborating with multidisciplinary teams to improve services and reduce delays.
• Coordinates referrals to social workers, financial counselors, dietitians, and community resources, including transportation and support groups.
